What We Do
Services Delivered with Expertise and Care
We provide individualized guidance, coordination, and practical support so individuals affected by stroke or brain injury can access rehabilitation services and move forward with a clear plan.
Assessment and Care Planning
We begin by understanding each person’s health status, functional challenges, home environment, support system, readiness, and goals. This assessment helps identify needs and develop a practical plan for accessing appropriate services and supports.
Intensive Rehabilitation Support
We help individuals explore and access specialized rehabilitation programs that address significant functional challenges. Support may include guidance in identifying programs, coordination of participation, and financial assistance when available.
Sustainment Program
Ongoing support for individuals who wish to continue rehabilitation after intensive services or who are unable to participate in full-time programs. This may include periodic therapy sessions, guided activities, caregiver education, technique refinement, and monitoring of functional changes.
The program may also include partnerships with colleges and universities, where supervised students in rehabilitation-related fields assist under professional oversight, expanding available support for participants while providing educational opportunities.
In-Home Integration Support
Guidance on adapting the home environment to support safety, accessibility, and daily function. This may include recommendations for layout changes, safety features, adaptive equipment, and ways to incorporate therapeutic strategies into everyday activities.
Equipment and Orthotics Support
Assistance in identifying and obtaining orthotics, adaptive devices, and specialized equipment that support functional improvement and independence.
Financial Assistance
When funding is available, we provide subsidies to help reduce financial barriers to rehabilitation services and related supports, including travel, lodging, equipment, and home modifications.

Transparency & Stewardship
We strive to direct at least 70% of every dollar toward services and support for individuals affected by stroke or brain injury, including rehabilitation access, equipment, and coordination. The remaining funds support the responsible
