Assessment & Planning Care

Filling the Post-Acute Care Void

After a stroke or brain injury, many people leave the hospital without clear guidance about what to do next. Families are often left to figure out rehabilitation options, safety needs, and support on their own.

We provide a personalized assessment to understand each person’s health status, abilities, goals, home situation, support system, and financial concerns. Based on this information, we help create a practical care plan and connect individuals with services and resources that match their needs.

This process helps families make informed decisions and move forward with greater clarity and confidence.

PROGRAM 2

Intensive Rehabilitation Therapy

Recovery, Not Just Compensation

Some individuals want to participate in specialized rehabilitation programs that are not available near their home. Travel, cost, and logistics can make access difficult.

We help participants explore intensive rehabilitation options and, when possible, provide assistance to support participation. This may include guidance, coordination, and financial help for expenses such as travel, temporary lodging, orthotics, or adaptive equipment.

Support is tailored to each person’s goals, readiness, and circumstances.

Program 3

In-Home Support

Where Therapy Meets Daily Life

Progress in therapy is easier to sustain when the home environment supports safety and daily practice.

We help families make changes that allow individuals to function more safely and independently at home. This may include recommendations for accessibility improvements, organization of living spaces, safety features, adaptive equipment, and ways to incorporate therapeutic strategies into everyday activities.

These adjustments help reinforce progress made in rehabilitation and support daily function.

PROGRAM 4

Sustainment Program

Bridging the Critical Gap

Many individuals need ongoing support after intensive rehabilitation, while others may not be able to participate in full-time programs.

Our Sustainment Program helps people continue building skills and preserving progress through periodic therapy, individualized incorporation plans, caregiver education, technique refinement, and monitoring of changes over time.

This program can serve as continued support after intensive services or as a starting point for individuals exploring rehabilitation options.

Subsidized Services

Making Recovery Accessible

Our Commitment: Financial barriers will never stand between a survivor and the therapy they need. After assessment, we develop your six-month care plan and determine subsidization levels based on demonstrated commitment and financial circumstances.

What We Subsidize: Occupational and physical therapy • Home modifications and equipment • Orthotics and adaptive devices • Referral services from partner specialists
