Eligibility Requirements

We proudly provide select services, support, and/or financial assistance to eligible adults in California who meet the following criteria. For consideration, an applicant must:

• provide evidence of U.S. citizenship and legal residency in California;.
• be diagnosed with stroke or brain injury;
• demonstrate that health insurance benefits related to Physical Therapy (PT) and Occupational Therapy (OT) have run out;
• demonstrate a continued need for PT or OT to grow improvement;
• demonstrate personal financial hardship preventing him/her from paying for additional needed therapy, and;
• demonstrate a desire and commitment to proceed with therapy, services, support and overall recovery process.
